This is a unique gag as the part that is designed to sit in the mouth is shaped like an O-ring. The O-ring has an outer diameter of 2 inches and sits behind the teeth, so it does not fall out.

The spider mouth gag is held in place by a strap that goes behind the head. The strap is adjustable and fits a head circumference of up to 22 inches. If necessary, you can make more holes in the strap.
